New Poison Centre Consultation Form for Health-Care Professionals
In an effort to help streamline your call experience with the Poison Centre, we have created a 2-page documentation tool that can be used during your consultation, at your discretion.
The first page of this fillable form is meant to be completed prior to calling the Poison Centre to optimize efficiency during the call. It includes relevant patient information, presentation, and treatments already initiated by your facility.
The second page covers the recommendations from the Poison Specialist you speak with and is organized into several sections: identified priorities, specific labs to be drawn, and treatment recommendations.
This form is meant to be a useful tool; it is not meant to serve as an order set, nor should it be considered an official chart document. All recommendations from the Poison Specialist must be reviewed and verified by the Most Responsible Physician (MRP).
This form will be available for download on our website, ontariopoisoncentre.ca, under Patient Care Resources.
