Patients over the age of 12 can sign up for their own MyChart account. Patients in this age group can decide to also grant access to someone else. This is known as proxy access. For patients under the age of 12, only parents and legal guardians can act as proxy.
A proxy is a parent, caregiver or guardian who has access to your health record. Proxies see everything a patient can see in MyChart. Once you have your own MyChart account, it's up to you who has access to your health information. You can control proxy access by adding or removing proxies by following the steps below.
To add a new proxy:
- From the MyChart menu, click Sharing Hub and select Family member, close friend, or caretaker.
- Click Manage ongoing access to your MyChart account to give proxy access to a parent or caregiver.
- Select Invite friends or family. Fill out all of the necessary fields of the form and click Send Invite.
- They will receive an email notification inviting them to access your record. You can revoke their access at any time.
To remove an existing proxy:
- From the MyChart menu, click Sharing Hub and select Family member, close friend, or caretaker.
- Click Manage ongoing access to your MyChart account to manage proxy access.
- Select Remove under the Proxy’s name.
- A confirmation bubble will pop up asking Are you sure you want to remove access to your record for Mom Mychart at The Hospital for Sick Children (SickKids)?
- Click Remove. They will not receive a notification when you do this.
There are two ways to sign up to MyChart :
- Ask registration staff at your appointment to send you an activation code to enroll.
- Fill out the appropriate MyChart Access Request Form. The Health Information Management team will review your application. If we approve your application, we'll send you an email with your activation code and instructions on how to login.
We are committed to protecting the privacy and security of your health information. Access to MyChart is safeguarded through secure activation codes, unique usernames, passwords, two-factor authorization and additional security measures. Each user is responsible for their own password, and no one can access an account without it.
All personal health information in MyChart is stored securely in Ontario and protected with encryption to ensure a high level of security. MyChart has been assessed to confirm it meets the requirements of Ontario’s Personal Health Information Protection Act (PHIPA) and other applicable privacy laws.

Currently, MyChart is available in both English and French.
MyChart is compatible with screen readers and other accessibility tools that help visually impaired users navigate the site.
SickKids recommends using NVDA (NonVisual Desktop Access), a free screen reader for Windows computers. After downloading and installing NVDA, you can navigate through MyChart using your keyboard and have page content read aloud.
To get started, use the link above to download NVDA. Once installed and running, place your cursor on different areas of the page or use keyboard navigation commands to have the screen reader announce content and page elements.
If you use a mobile device, you can also access MyChart using your device's built-in screen reader features, such as VoiceOver (iPhone/iPad) or TalkBack (Android).
To access your entire health record, you will need to submit a request to Health Information Management (formerly Health Records). You can find more information about the request process, including how to submit a written request and any associated fees on the Health Records Inquiries page on sickkids.ca.
